2. How do you refer youth to the center?

To schedule an appointment for a youth to visit the Shelby County Youth and Family Resource Center, please complete this form or call or visit the Youth and Family Resource Center. 

Upon completion of the form, you will be contacted by staff to finalize the appointment.

3. When should you refer a youth?

When youth are struggling at home, community, or in school- or at risk of justice system involvement.

Youth ages 10-17 that reside in Shelby County

5. How do I make a referral?
