The Aging Commission of the Mid-South provides an alternative to nursing facility placement through its Home & Community-Based Services (HCBS) Programs. The Aging Commission completes in-home assessments on potential clients to identify their care, service needs and their eligibility.

OPTIONS for Community Living Program

OPTIONS for Community Living is a state-funded program created to provide the elderly and adults with disabilities home and community-based service choices. Tennessee families want to care for loved ones at home for as long as possible.  The purpose of the OPTIONS program is to enable consumers to live independently in their own homes or in a community setting. The OPTIONS Program allows ready access to community resources to improve quality of life for consumers and caregivers. The OPTIONS Program promotes increased planning coordination and consumer choices.  

Program Eligibility

To be eligible for the OPTIONS program, an individual must:

  • Be a resident of Tennessee.
  • Be age 60 or older OR
  • Be age eighteen (18) years or older with a documented assessment of a physical impairment. 
  • Must meet Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) limitation requirements.

There is no income eligibility requirement for this program; however, there is a cost share requirement if your income is above a certain level.

What services are available?

The services funded through the state for OPTIONS include the following:

  • Homemaker services
  • Personal care
  • Home-Delivered meals

Family Caregiver Support Program

The National Family Caregiver Support Program was authorized by Congress and provides program resources and assistance for family caregivers, grandparents, and older individuals who are relative caregivers.   Information, assistance and counseling can be provided to any caregiver; however, respite and supplemental services are limited to caregiver support for older individuals who are unable to perform at least two activities of daily living or require substantial supervision.

Program Eligibility for Family Caregiver

To be eligible for the National Family Caregiver Support program:

  • Adult family members (age 18 years or older) or other adult informal caregivers providing care to individuals 60 years of age and older. 
  • Adult family members or other adult informal caregivers providing care to individuals of any age with Alzheimer’s disease and related disorders.
  • Grandparents and other relatives (not parents) 55 years of age and older who are providing care to children under the age of 18 years.

What services are available?

  • Information about available services
  • Assistance in gaining access to supportive services
  • Individual counseling, support groups, caregiver training
  • Respite care – to give the caregiver assistance with care giving responsibilities
    • Personal care
    • Homemaker
    • Adult Day Care
    • In-Home Respite

If you provide care for an elder family member in Fayette, Lauderdale, Shelby or Tipton County, there are many resources to help you. Caregiver education, support groups, individual counseling and in-home services may help you care for your loved one and maintain your own quality of life.

Older Americans Act (OAA)

The Older Americans Act provides federal funds to pay for home and community-based long-term care services for older adults, generally 60 and older, and their families. The focus of these programs is to help older adults remain in the community as independently as possible.  Older Americans Act services are provided through state and local agency networks known as the Aging Network.

Program Eligibility

To be eligible for the OAA program, an individual must:

  • Be a resident of Tennessee
  • Be Sixty (60) years of age or older
  • Must meet Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) limitation requirements.

There is no income eligibility requirement for this program; however, there is a cost share requirement if your income is above a certain level.
