Thank you for your participation in the Shelby County Board of Commissioners Community Enhancement Grant Program which was reauthorized to provide assistance to certified 501 (c) (3) non-profit organizations and charitable organizations.
In accordance with the recent auditing requirements, please complete the Community Enhancement Grant Program Year End Report. This form must be completed and should include a detailed accounting of how all grant funds expended to your organization were utilized by the award recipient. Additionally, please ensure that the Chief Financial Officer or Treasurer for the organization certifies the accuracy of the detailed itemized information being provided.
Please complete this report if you received funds during the 2017/2018 fiscal year before proceeding to apply for any additional funding in the current fiscal year 2018/2019. Failure to do so could limit or suspend your opportunity to receive future grant funds from our office.
