Subgrantee Internal Controls Questionnaire
Subgrantees are to complete the I/C Questionnaire upon the execution of a grant-related contract or agreement, then submit the completed form to their Shelby County Government project coordinator.

Subgrantees shall have policies and procedures in place and accessible upon request for each grant project that is awarded by Shelby County Government.

Grant Funded Personnel Time Sheets (To be completed for grant funded staff)
Compliance with Equal Services Provider Laws, Including Title VI
Federal laws prohibit grant recipients from discriminating on the basis of race, color, national origin, religion, sex, or disability in funded programs or activities, not only in respect to employment practices but also in the delivery of services or benefits. Federal law also prohibits funded programs or activities from discriminating on the basis of age in the delivery of services or benefits. NOTE: Shelby County Government subgrantees shall have a Title VI poster displayed in conspicuous areas wherever services are provided.
All subgrantees must complete a Title VI Self-Survey for each subcontract or legal agreement that is executed by Shelby County Government. A signed copy is to be placed in the subgrantee's project file and made available to auditors/monitors upon request. After completion the subgrantee is to provide a signed copy of the self-survey to the Shelby County Government project coordinator.

known as SAM)
2) Code of Federal Regulations (CFR) and Office of Management and Budget (OMB) Guidance
that applies to the particular subgrantee and grant program. This includes uniform
administrative requirements, cost principles and audit.
3) Notice of Grant Award or Agreement, also known as the legal agreement or contract
between the grantor or primary funding agency and Shelby County Government including
any terms, conditions or references cited within the document.
State Grant Awards
1) Notice of Grant Award or master agreement, also known as the legal agreement or contract
between the grantor or primary funding agency and Shelby County Government
including any terms, conditions or references cited within the document.
2) Specific guidance that is issued with the grant program that is normally stated within the
Grant Agreement.
3) Code of Federal Regulations (CFR) and Office of Management and Budget (OMB)
Circulars. Adhere to the CFR whenever the grant activity is not addressed in
the agreement or program guidance.
