If you answered "YES" to at least one of these questions, you may qualify for Community Services Agency MEDICAL PRESCRIPTION PROGRAM.

This program is for Memphis and Shelby County residents whose monthly income falls below 125% of Federal Poverty Income Guidelines.  Eligible households can be served once per year and receive up to $300 per household.

NO CRISIS NECESSARY

Prescription must be new or refillable and non-narcotics
