The current rate schedule reflects premiums effective as of July 1, 2012.

The County will continue to offer the CIGNA OAP In Network (OAPIN-HMO), CIGNA Open Access Plus (PPO), and CIGNA Choice Fund HRA plans.  You can visit www.mycigna.com or contact CIGNA Member Services at 1-800-558-7453 for more plan information.

"Health Screenings"
Since the County's health plans are non-grandfathered, The Affordable Care Act requires the County to offer eight (8) additional screenings at no extra cost to the employee.  These include:
        - Well-woman visits
        - Gestational diabetes screenings that help protect the mother and child from
one of the most serious pregnancy-related diseases
        -  Domestic and interpersonal violence screening and counseling
        -  FDA approved contraceptive products
        -  Breastfeeding support, supplies and counseling
        -  HPV DNA testing for women 30 or older
        -  Sexually transmitted infectious counseling
        -  HIV screening and counseling

 "Contraception Mandate"

Under the Affordable Care Act, health insurance reform legislation, all FDA approved contraceptive methods and education counseling are covered for all women with reproductive capacity.  This mandate is effective for Shelby County plans January 1, 2013.  For more information, visit the federal government website at www.healthcare.gov.

REMINDERS:

ü  Preventive Care:  Preventive care, under the Affordable Care Act, is covered 100%.  Remember, preventive services are not subject to deductible or copays.  Please check with your physician to determine which services are considered preventive.  This provision is applicable to all family members.

ü
Age 26 Adult Dependents:   Dependent coverage is offered to enrollees’ adult children until age 26, even if the young adult no longer lives with his/her parents, is not a dependent on a parents’ tax return, or is no longer a student.  The Affordable Care Act applies to both married and unmarried young adult children, although their own spouses and children are not eligible for coverage.  Employees must complete a CIGNA employee/retiree health care plans enrollment/change form to enroll dependents 19 up to age 26 during open enrollment or if there is a life style/qualifing event change.  Employees must submit with the enrollment form a copy of the dependent’s birth certificate and/or other pertinent documentation to establish eligibility and enroll the adult child on the health plan.

NOTE:  If you currently have an eligible age 19 to 26 adult dependent enrolled on your plan, you DO NOT have to re-enroll that dependent during open enrollment.

ü  Ineligible Dependents: It is the employee’s responsibility to complete and submit to Employee Benefits a CIGNA employee/retiree health care plans enrollment/change form to delete/drop any ineligible (e.g., ex-spouse) or overage Age 26 adult dependent from coverage. Failure to remove any ineligible dependent will result in you being responsible for any claims paid on any ineligible dependent.

Important Medical Information
During the year, prior to open enrollment, employees who experience a life status/qualifying event change have 31 days to complete proper forms and provide the required proof to enroll self and/or eligible dependents in all applicable benefits.

It is the employee’s responsibility to complete proper forms to remove ineligible dependents from all benefits.

We encourage you to carefully read all information concerning your County benefits and visit the carriers’ websites and call their Customer Service centers for more information.
