2008 - 2009 School Entry Requirements

Toward School Success


Please read the information below completely

Students entering public and private schools for the first time in the Memphis and Shelby County area are required to provide the following:

- certified copy of birth certificate,
- social security number,
- proof of medical examination and
- proof of required immunizations
- proof of residence in school district


Tennessee immunization requirements for school entry include: *

Child Care Attendance
Children attending child care must be immunized age-appropriately according to the schedule below:

Age of Child

Number of Doses Required

2-3 months

1 DTP/DtaP/DT, 1 Polio, 1 Hib

4-5 months

2 DTP/DtaP/DT, 2 Polio, 2 Hib

6-11 months

3 DTP/DtaP/DT, 2 Polio, 3 Hib

12-15 months

3 or 4 Hib*, and 1 MMR given on or after the first birthday

12-18 months

4 DTP/DtaP/DT, 3 Polio, and 1 Varicella

4-6 years

4 or more DTP/DtaP/DT, 4 Polio, 4 Hib*, 1 Varicella, and 2 MMR given on or after the first birthday

*If child starts Hib at 15 months through 4 years of age, give just one dose of Hib vaccine. The number of doses depends on the type of vaccine used - 1 dose on or after 1st birthday.

School Attendance K-12

DTP: All students must have at least four (4) doses.
One (1) must be given after the 4th birthday.

Polio:
All students must have at least four (4) doses -  for all grades.  If only 3 doses, 1 must be after 4th birthday.

MMR:
All students K thru 12th grade must show proof of having received two (2) doses of MMR (measles, mumps, rubella vaccine) after their 1st birthday - for all grades.

Hepatitis B: Proof of adequate immunization is required for Kindergarten and 7th grade because:

- Effective July 1, 1999, proof of adequate immunization against Hepatitis B was required prior to entry into kindergarten.

- Effective July 1, 2002, proof of adequate immunization against Hepatitis B was required prior to entry into the seventh grade.

If student had this series of Hep B shots as a baby, the shot does NOT need to be repeated for 7th grade.

Varicella (Chickenpox): One (1) dose given after 1st birthday is required for Kindergarten.

*Children in pre-school classes at schools or in child care facilities have different requirements.

College

Two (2) doses are MMR are required for full-time students attending college in Tennessee if they were born after 1957. Parents and students should also consider the need for the meningococcal vaccine, particularly if the student will be living in a dormitory.

REQUIRED IMMUNIZATIONS for school attendance are available NOW at all Health Department Public Health Clinics; Health Loop Clinics; Immunization Clinic, 814 Jefferson Ave; DHS Immunization Clinic, 3320 Jackson Ave., and private health care providers.

REQUIRED CERTIFIED COPIES OF BIRTH CERTIFICATES can be obtained at two (2) convenient locations:

  • Monday - Friday from 8:00 a.m. - 4:00 p.m. at the Hickory Hill Vital Records office, 6590 Kirby Center Cove, (901-362-1015) and
  • Main Health Department location, Room 101, 814 Jefferson Ave. (901-544-7608) OR
  • From the birth certificate section of this website.

Birth Certificates for children born in Shelby County cost $7.00 for the first copy of short form and $4.00 for each additional copy when purchased at the same time.  The first copy of the long form is $12.00 and $4.00 for each additional copy.

The Hickory Hill location usually has shorter lines and therefore clients can get the number of birth certificates needed in a shorter period of time, than at the main Health Department location.  Parents are encouraged to get copies of birth certificates early as the lines get longer the closer it gets to school re-opening in the Fall.
