The Memphis and Shelby County Health Department's TennCare Presumptive Eligibility Unit in collaboration with the Department of Human Services (DHS) has designed the first and only one-stop TennCare Presumptive enrollment system in the state of Tennessee.The purpose is to provide an expedited entry point for women to receive early prenatal care and/or cancer treatment or diagnostic medical services by way of enrollment into the TennCare Presumptive Medicaid program.  It also provides an opportunity for the women to complete the required Medicaid application process at the same time, therefore, eliminating the need for a visit to their local DHS community office.

WHO CAN GET TENNCARE?

    PRESUMPTIVE ELIGIBILITY

Presumptive Eligibility is temporary Medicaid coverage to pregnant women or women diagnosed with breast or cervical cancer whose income is at or below the specified percentage of the poverty level. This allows the woman to receive health care more quickly to help ensure a healthy lifestyle.

    PRENATAL PRESUMPTIVE PROGRAM

There are four basic requirements that must be met in order to be eligible:

- You must show proof of pregnancy by providing a signed and dated pregnancy confirmation form.
- Your gross monthly income must be at or below 185% of the federal poverty level.
- You must be a resident of Tennessee with a Tennessee driver's license or other state I.D.

- You must have a social security card and be a U.S. citizen or qualified alien.
- You must have a valid birth certificate.

     BREAST & CERVICAL CANCER PRESUMPTIVE  PROGRAM

There are six basic requirements that must be met in order to be eligible:

  1. You must be diagnosed with breast or cervical cancer or pre-cancerous condition.
  2. Your gross monthly income must be at or below 250% of the federal poverty level.
  3. You must be a resident of Tennessee with a Tennessee driver's license or other state I.D.
  4. You must have a social security card and be a U.S. citizen or qualified alien.
  5. You must be uninsured or your insurance does not pay for this condition.
  6. You must be under 65 years of age.

If you already have Medicaid/Medicare/TennCare you cannot apply for Presumptive Eligibility.

Coverage is temporary and lasts for 45 days. In order to continue Medcaid coverage, an application with the Department of Human Services (DHS) must be completed.

  TN. BREAST AND CERVICAL CANCER SCREENING PROGRAM

The Tennessee Breast and Cervical Cancer Screening Program provides clinical breast exams, mammograms, and Pap tests for eligible Tennessee women free of charge.  General eligibility guidelines are women who meet income guidelines and either have no insurance, or insurance that does not pay for mammograms or Pap tests.

There are four (4) basic requirements that must be met in order to be eligible for the mammogram screening:

  1. Age Requirement
    • Ages 50 - 64 years
    • Ages 40 - 49 years - must have a first degree relative with breast cancer, personal history of breast cancer or a positive clinical breast exam
    • Ages 18 - 39 years - must have one or more symptoms:  breast lump, nipple discharge, skin or nipple change
  2. Your gross monthly income must be at or below 250% of the federal poverty level
  3. You must be a resident of Tennessee with a Tennessee driver's license or state I.D. 
  4. You must be uninsured or your insurance does not pay for this condition.

There are four (4) basic requirements that must be met in order to be eligible for the Pap test screening:

  1. Age Requirement:
    • Age 40 to 64 years
    • Age 18 to 39 years - only covers further diagnostic services, inital Pap is not included.
  2. Your gross monthly income must be at or below 250% of the federal poverty level.
  3. You must be a resident of Tennessee with a Tennessee driver's license or state I.D.
  4. You must be Uninsured or your insurance does not pay for this condition.
