How to Apply

  •  Must Live In Shelby County
  •  Complete Pre-Registration Form (Pre-Registration Form)
  • Registration Forms can be mailed to the Corporate Office address located at the top of the form or completed and taken to the center site of your choice.
  •  Children Must Be 3 or 4 Years Old By September 30th 
  •  Age Verification:  Birth Certificate 
  •  Income Verification:  W-2 Forms, Federal Tax Return For Previous Year, Letter From      Employer, Letter From The Department of Human Services, Social Security Office, and/or Current Check Stubs
  • Proof Of Address
  • Your Child's Social Security Card
