 2008 Open Enrollment

 
 


This year open enrollment will be held November 3 - 14, 2008.  The enrollment is for Medical, Dental, Life Insurance, Flexible Spending Accounts (FSA), and AFLAC.  IF YOU ARE NOT MAKING ANY CHANGES TO YOUR MEDICAL, DENTAL, LIFE OR AFLAC, THERE IS NOTHING FOR YOU TO DO.  HOWEVER, IF YOU ARE CURRENTLY ENROLLED IN FSA, YOU MUST RE-ENROLL.  All changes will be effective January 1, 2009.

 

MEDICAL

 

This is the only time that you can add or drop dependents without a qualifying status change.  Additions will require proper documentation, i.e. marriage license, birth certificate.  No proof is required to drop a dependent during open enrollment.  You must delete over aged dependents (19+ years, unless full time student) from your coverage.

 

Shelby County Government offers 3 medical programs - CIGNA OAP In Network (OAPIN/HMO), CIGNA OAP/PPO (OAP), and CIGNA Choice Fund HRA. 

 

PLEASE NOTE:  THE DEDUCTIBLE FOR THE CHOICE FUND HRA PLAN WILL BE $1500 EMPLOYEE, $3000 EMPLOYEE+1, AND $4500 FAMILY EFFECTIVE JANUARY 1, 2009.  THE AMOUNTS THAT SHELBY COUNTY WILL CONTRIBUTE TO THE CHOICE FUND HRA PLAN, $650 EMPLOYEE, $1300 EMPLOYEE+1, AND $1950 FAMILY WILL REMAIN THE SAME.

 

You must complete an enrollment form if you are making changes.  A summary of the medical benefits is included in "Your Benefits" booklet and on the website.  Medical premiums are also available on the website.  You can contact CIGNA Member Services at 1-800-558-7453 for more plan information.

 

 Prescription Drug:   There are no changes in prescription drug benefits.  If you enroll in CIGNA OAP In Network (OAPIN/HMO) or CIGNA OAP/PPO (OAP), Medco will be your pharmacy provider and will mail your prescription ID card.  You can contact Medco Member Services at 1-800-711-0917.  If you enroll in the CIGNA CHOICE FUND HRA, CIGNA Tel Drug will be your pharmacy provider. You can contact CIGNA Member Services at the above number.

 

 

FLEXIBLE SPENDING ACCOUNTS (FSA)

 

ADP is County's Flexible Spending Account (FSA) administrator.  If you are currently enrolled in a FSA and wish to participate in 2009, YOU MUST RE-ENROLL.  FSAs let you save taxes by lowering taxable income on out-of-pocket health and dependent care expenses.  The minimum contribution for a Medical FSA is $25.00 per payday, and for a Dependent Care FSA, $50.00 per payday.  Enrollment forms and Frequently Asked Questions are available on County's websites.  You can visit ADP's website at www.flexdirect.adp.com or contact ADP Member Services at 1-800-654-6695.

 

CURRENT FSA PARTICIPANTS - REMEMBER, YOU HAVE UNTIL MARCH 31, 2009 TO FILE A CLAIM FOR 2008 FSA EXPENSES OR YOU WILL LOSE ANY MONIES THAT HAVE NOT BEEN SPENT.

               

DEBIT CARD - Effective January 1, 2009, ADP will issue a Visa Debit Card for employees who enroll in the Medical FSA.  You will no longer have to pay out of pocket and wait for reimbursement of eligible medical expenses.  The card CANNOT be used for Dependent Care expenses.  Visit County's website for more information.   

 

LIFE INSURANCE          

 

The Standard Insurance Company is the County's life insurance carrier.  Please obtain a copy of the life insurance plan 'Basic and Additional Life Coverage Highlights" from you Human Resource Representative or visit the website.  During this enrollment, you may make beneficiary, additional life and dependent life insurance changes by completing an enrollment and change form.  If you are enrolling or increasing your Additional and/or Dependent Life coverage, you must complete a Medical History Statement for coverage approval by Standard Insurance. 

 

NOTE:  There will be a change in premium when an employee moves to the next 5 year age band.  The change will be made on January 1 of each year.

 

As a participant in County's group life insurance plan through Standard Insurance, employees are eligible for MEDEX Travel Assist, free of charge.  This is a program designed to help you respond to medical care situations and many other emergencies that may arise during travel. You may obtain a brochure from your Human Resource Representative.

 

 

SHORT TERM DISABILITY

 

This plan is underwritten by the Standard Insurance Company.  You can enroll or cancel Short Term Disability at any time throughout the year.  Forms may be obtained through your Human Resources Representative or County website.

 

                                                                     

DENTAL

 

You will receive an enrollment booklet mailed to your home address from Humana/CompBenefits, formerly CompBenefits, for any changes you wish to make to your dental coverage.  You must delete over aged dependents (19+ years unless full time student) from your coverage. A dental enrollment and change form are included in the booklet and on the website. 

NOTE:  The PPO Options III & IV provider networks have been expanded.  Please refer to the Humana/CompBenefits dental booklet.  If you enroll in the dental plan, Humana/CompBenefits provides a discounted vision plan (see dental booklet).
