1. What does Women, Infants, and Children (WIC) program provide?

WIC provides the following:

• A food voucher to be redeemed at the grocery store


• Breastfeeding support


• Linkage to other healthcare, ie: well child exams, immunizations, lead screening


• Nutrition education

2. What do I need to bring when I sign up for WIC services?

The following information will be needed when applying for WIC:

• Crib card from hospital or birth certificate (sixty days old or less)


• Proof of identity and proof of person receiving the vouchers


• Proof of income


• Proof of residency


• Shot record for infants and children

3. Is a medical exam required for WIC services.

WIC accepts physician referral information if not older than 60 days. A WIC exam can be done at the clinic. A WIC exam is:

• Due date for pregnant women


• Height and weight


• Hematocrit or hemoglobin (if over nine months of age)

WIC participants must be present at each certification.
